Options B E can be discarded. Among A, C and D ,D wins because while A and C cast a doubt on the effect of the drug, D enforces that the drug works but in ways it is not supposed to. Hence paradoxical

both manic and depressive = both POSITIVE (in the sense too active maniacal) and negative ( depressive or slow inactive) lead us to a paradox because the drug works in both spectrums
