If "distinct" diseases is already mentioned, usage of clear-cut is not very useful. This led me to also consider recalcitrant as an option. How do we reject recalcitrant ?

Let's analyze the context for the three blanks:

(i) The sentence mentions an effort within psychiatry to do something to medical diagnosis and instead treat behavior dimensions. The word should suggest changing the approach to medical diagnosis, likely to improve or rethink it. Suitable options here are "abandon" (giving up on it) or "revamp" (reworking it). Given the context of an effort to replace diagnosis focus with behavior dimensions, "abandon" or "revamp" could fit. "Motivate" and "canonize" are less fitting.
(ii) Realistically, this project won't do something to the diagnostic edifice-likely it won't completely tear it down or remove it. Words like "dismantle" or "bolster" might fit. "Dismantle" means to take down, "bolster" means to support. Since it says "too much depends on the fiction" of distinct diseases, the project won't fully remove it, so "dismantle" (remove) fits well in the negative sense.
(iii) The "fiction of _______ biologically distinct diseases" means the classification of diseases as clearcut categories, but this is described as fiction. The best fit is "clear-cut".

So the best answers are:
