The sentence requires a word synonymous with lack of respect. The first part of the sentence states that, people are usually critical of doctors. However they do not treat their personal doctors the same way.

Thanks Sandy. I do the same mistake always. I don't understand the sentence properly sometimes. This is happening in both TC and SE. Thanks for explaining the meaning.
