Rather find out where is stated implicitly and explicitly that X is a dog (which I have a hard time to figure out), to me the question seems truncate.

The sentence begins with Despite, which sets up a contrast between the two ideas present in the sentence. The first part of the sentence is expecting us to describe her painful arthritis. Since the second part of the sentence describes Maj as pleasant, the adjective or adverb describing painful arthritis has to produce the opposite sense. Mildly reduces the painful effect of the arthritis and hence is not the right word. Sympathetically does not make sense. Therefore, chronically is the best word. Now the second blank expects us to provide a word that goes well with pleasant. Enervated means to be drained of energy and vitality and hence is the wrong word, and acute, which means intense or sharp (faculties) is unrelated. Agile, means dexterous or nimble is the only choice. It is contrasting to note that someone who is chronically in pain due to arthritis is also agile.
