Let there be 100 patients. Number of patients improving their symptoms 90.
Number of patients developing side effects after improving symptom is 60% of 90 = 54.
Number of patients not improving their symptoms at all 10. So even if they all developed side effect from the treatment the total number of patients with side effect would be 64.
Hence A is the right answer.
