Essentially
1) The language of the question
2) The twists inside of it
3) the concepts tested if they are multiple or a simple concept
For example: if the question states that x =0 is one thing but if x is \(\neq\) zero means that x could be any value but zero and this is a HUGE difference to make the question harder.

Many times what make a question difficult is how "layered" the question is. For example, perhaps you see a Geometry question that tests right triangles but also quadratic equations and roots rules. This is why taking a topical approach to your studying is so important, so you can master each topic one by one. Feel free to reach out with any further questions.
