Blank (i)Blank (ii)
A) mistakesD) endorsed
B) experimentsE) misinterpreted
C) ritualsF) reversed

i firstly choose A but for second blank, the way i interpret the information distract my choice for both blanks and choose E
as '' Even in cases where findings are not officially retracted, they are sometimes (ii)___________ — definitively shown to be false, and thus no longer valid pieces of scientific information.
'' i interpreted it as that the findings which are not revised , they are misinterpreted ,shown to be false and no longer valid pieces of scientific information (coz no further revised or work on it ).

can you please explain where I'm getting it wrong

Science proceeds in stops and starts. Given the inherent uncertainty in creating new knowledge, errors will be made, and have to be corrected. Even in cases where findings are not officially retracted

The first line of the passage and the line above tells us quite a lot about the gist.

Reversing something is a key part of the scientific process. Considering the nature of scientific work, errors are inevitable and have to be CORRECTED. Even in some cases you cannot take it back, you have to instead reverse it. There is a hint for the second blank after the dash. This blank was tricky as you pointed out that could mean misleading, but going back to the first line of the sentence, it is clear that the word has to be "reverse". The first blank has to be mistakes as it is synonymous to errors.

How can you switch a mistake? That is why I avoided (A)
